These are low-stakes prompts to choose next teaching. Keep the educator key away from first responses. Provide broad fixed/raised strips, high-contrast column window, AAC, neutral reading/scribing, drawing, pointing and pass. No device, hearing, singing, partner, instrument or personal story is required. Optional approved sound happens after the first independent paper answer and is a separate observation.

For each column from 1 to 4, choose one summary: UPPER ONLY, LOWER ONLY, BOTH, or QUIET. BOTH means two X in one vertical column; QUIET means two REST there. Show the ordered four-label key or an equivalent child-explained symbol key. This is invented paper music, not a real window signal.
Save first: the four labels in order, which vertical cells were read, child explanation and response route. Do not give the four-label solution before saving. The plan alone cannot show that the child heard layered sound.

In the lower line only, change the X at count 2 to REST. Keep lower counts 1, 3 and 4 as shown. Make the complete revised two-line score, then point to any column that still has two X together. No one has to play two sounds at once.
Save first: both full four-place lines, the one changed lower cell, remaining together column, child's explanation and route. A correct score is not evidence that anyone sounded or coordinated it.
